Consultation for addiction medicine programs within primary care or host setting
I am the program lead for the medication for addiction treatment program within Clackamas Health Centers. I started this position in 2018 when the program had no coordinator and around 75 patients. We are now at nearly 300 patients with two LCSWs leading the work and coordinate with an interdisciplinary team to ensure patients are receiving trauma-informed care that meets criteria for best practices. This work includes program development with a harm reduction and person centered lens. I coordinate with my team to develop protocol, policies, and paperwork that is trauma informed and meets criteria for our legal and oversight teams. I work with our team to develop and maintain community partnerships. These include working with corrections and community corrections to coordinate and offer wrap around care for those in these systems. I have developed, with our Epic team, reports that track data to ensure federal regulations are being met, track refills and appointments to ensure patients are engaged and receiving appropriate level of care.
